Restarting Treatment After a Gap and How Dose Adjustments Are Managed

Patients sometimes have questions about why a lower dose may be recommended after a pause in treatment and how future dose adjustments are handled.

This information is provided for general educational purposes only. All dosing decisions are made by licensed providers.

Restarting After a Treatment Gap
If there has been a break in treatment, a licensed provider may determine that restarting at a lower dose is appropriate. This decision is based on clinical review and individual medical factors, including the length of time since the last dose and a patient’s treatment history.

Restarting at a lower dose is not automatic for every patient and is evaluated on a case-by-case basis by the provider.

Considering a Different Restart Dose
In certain situations, a provider may consider a different approach if there is recent documentation showing prior treatment and tolerance. Any such determination is made solely by the provider after reviewing relevant medical information and records.

Patients should not attempt to resume a prior dose without provider guidance.

Switching Care Without a Treatment Gap
If a patient is actively receiving treatment and transferring care without a break, a licensed provider will review the available records to determine whether continuation of a prior plan may be appropriate. Continuation is not guaranteed and depends on clinical review.

How Dose Adjustments Are Managed
All dose changes require provider review and approval. Changes are not automatic and are based on individual medical information and provider judgment.

If you have questions about restarting treatment, treatment gaps, or dose changes, please message your provider through the Patient Portal. The Care Team can also assist with routing your inquiry for appropriate review.
